315 health was founded in 2022 with a mission to change the delivery of healthcare. We believe that everyone should have confidence in the navigation of their health. Our registered nurses work fervently to anticipate the needs of our patients and connect on a personal level through continuous care programs like Remote Patient Monitoring, Chronic Care Management and Principal Care Management. We exist to coach people to their best health! 

The founders were inspired by the concept of shepherding others towards a healthier lifestyle as described in Jeremiah 3:15. 

"Then I will give you shepherds after my own heart, who will lead you with knowledge and understanding."

We believe our nurses will help nurture our patients through therapeutic relationships, personalized patient education and coaching, and regular communication and support.
